摘要:Expansion of hematopoietic stem cells holds great promise for the application of cord blood transplantation in adult patients. The most promising approach is the use of stem cell self-renewal factors such as homeobox (HOX) genes to expand stem cells. We have demonstrated in a clinically relevant nonhuman primate model that HOXB4 overexpression has a dramatic effect on expansion and engraftment of short-term repopulating cells and a significant, but less pronounced, effect on long-term repopulating cells. Surprisingly, 3 out of 4 long-term follow-up animals (monkeys and dogs) developed leukemia at ~2 years after transplantation and 1 died of MDS at 3 years after transplantation. Further analyses demonstrate the monoclonality of leukemia and the involvement of retrovirus insertional mutagenesis. From one of the leukemia animals, we created a canine leukemia stem cell line, which robustly engrafted immunocompromised mice and normal dogs. The establishment of the first large animal leukemia model provides us with unique opportunities to develop novel strategies for leukemia treatment. In the end, I will discuss on the challenges and opportunities of stem cell expansion, stem cell gene therapy and regenerative medicine.
